CCAT tunneling for mild defect

 

剛上完課就有 tunnel case，上課真會吸 case，能單純的當學生也蠻幸福的。

就以這個案例向 Otto Zuhr 致敬。

    這個案例 24 recession + thin phenotype

依 Zuhr 的 CCAT 做法，用 split-full-split prepare tunnel.

• midbuccal site 做 full thickness 超過 MGJ
• papillae 的 buccal flap 用眼刀打 partial。
• 過 MGJ 的 mucosa 用眼刀打 superficial partial 
• papillae base 也用眼刀弄鬆。

    

     取塊 FGG，去皮成 CTG，肉質很美麗

graft放在牙齒上定位，較厚的邊朝 margin，graft 的 surface 是關鍵。

positioning suture 從 25 穿到 23，將 graft 從開口最大的 23 塞進去。

兩邊再用 positioning suture將 graft 拉好到靠 coronal 位置。

兩邊 papillae 用 double sling suture，將 graft 跟 papilae 往 coronal 吊。

因為覺得 buccal site 的 flap 沒有很貼，又再加上 sling suture

比較只用 double sling suture 跟 sling suture

在 mid-buccal site 的 flap 比較高， flap 也比較密貼。

因為 Zuhr 喜歡將 graft 做 partial exposed，所以用 double sling suture 會很剛好。

     

這個案例因為 24 supra-eruption，graft 不易 exposed，所以我就加 sling suture 關緊了。
