Amputation - 時代的藝術

Amputation 似乎已經變成歷史的代名詞，卻偶有神奇效果

這個案例 mesial root 因為 vertical root fracture， amputation + crown 後

至今五年，也算是個小小的 long term follow 吧

五年 finding

• buccal gingiva recession
• crown、root、periodontal condition 都穩定

雖不常做，但大多切過的牙齒：牙周狀況很穩定

根據文獻，Amputation 後壞掉的原因，不是牙周而是 fracture。

這個案例很小心的撐了五年，希望努力下去。

操作細節

• 砍牙根時，要連同牙根上方支撐的 crown portion 都一同去除，將三根 root 的 molar ，修成像本來就是兩個根的 premolar
• 靠近 furcation 處的牙根，很容易切不乾淨，殘留到凹的牙根。務必要確保從 residual root 到 crown 都是很順的上去，將切根後的 emergency profile 做順。。
• 強烈建議在 micro 下進行，才可以看清楚有沒有修乾淨
• crown 型態要改變，occlusal table 的面積要縮小。cantilever portion 的 crown 不要咬到。

crown margin 下的部分，跟 implant emergency profile 設計原理一樣。

• 殘留的牙根，靠近 bone area 的區域，就是天然的 C zone
• subgingival 的牙根，及 subgingival 的 crown portion 是 B zone。有點 concave，外展角度盡量不要太大。
• gingival margin 部分就是 E zone，決定牙齦表現的型態。

對照假牙剛裝上去跟五年結果，buccal site soft tissue 有些許 recession

x ray 上，bone level 很穩定

案例心得

• amputation 好好做，有機會撐蠻久。
• 切根處的骨頭會凹陷下去，手術時做 ARP 及 ridge augmentation ，有機會達到更漂亮跟穩定的結果
